  • DHEA / DHEA-S (dehydroepiandrosterone): the "youth hormone"; its decline begins at age 25–30. The DHEA/cortisol ratio is an integrated indicator of age-related adrenal dysfunction.
  • Testosterone (total and free): in men, it declines by 1–2% per year after age 40. LC-MS/MS is critical for measuring low concentrations in women.
  • Estradiol (E2): LC-MS/MS ensures accuracy at low levels (postmenopausal women, men), where immunoassays produce artifacts.
  • Progesterone: a marker of ovulation and the luteal phase, with neuroprotective properties.
  • Aldosterone: a regulator of water and electrolyte balance, linked to cardiovascular aging.
  • Estrogen metabolites: 2-OH-E1, 4-OH-E1, 16α-OH-E1. The ratio of these metabolites is associated with cancer risk.

Saliva Testing: Free Steroids

Saliva contains only the free (non-protein-bound) forms of steroid hormones, which makes it an ideal matrix for assessing bioactive fractions. LC-MS/MS can measure cortisol, testosterone, DHEA and estradiol in saliva with high sensitivity.

For anti-aging clinics, saliva testing is especially attractive because sample collection is non-invasive. A series of samples collected throughout the day makes it possible to assess the circadian rhythm of cortisol, an important marker of stress adaptation whose disruption is associated with accelerated aging.
